Rationale for angiotensin II receptor blocker therapy in chronic heart failure

Ang II plays a significant role in the development of heart failure. ARBs may provide further benefits when compared with ACE inhibitors owing to their more efficient blockade of the RAAS. This hypothesis is being studied in clinical trials. Val-HeFT is designed to test the morbidity and mortality benefit of the selective ARB valsartan, 320 mg/day, in addition to usual therapy including ACE inhibitors and beta-blockers. The outcome of Val-HeFT will be related to neurohormonal responses, LV remodelling effects and quality of life benefits. The results of Val-HeFT should provide clear evidence on the efficacy of valsartan in the long-term treatment of HF and establish the mechanism of any favourable effect.
